Where can I get this G128 Macro program?

Or the macro numbers for the tool eye. I try to use the g68.5 sample program from the manual and tool tip is not going where I think it should be. I've got an Integrex 300y with 640-MT control. Any help would be greatly appreciated!!

If you are doing 3+2 work in EIA you need it. You should be able to get it from Mazak for free. You do not need any options to to 3+2, but to do 5 axis simultaneous you do.

I am wanting to do basically what the G68.5 says it will do in the book. I need to position "B", position "C" and mill using "X,Y & Z". Not really full 5 axis. Our support from Mazak had expired years ago, I don't have any contacts with Mazak.
